Diabetes

We are changing the way we organise diabetes care at Cults Medical Group. This will ensure that all Diabetes Healthcare Essentials* are addressed annually which leads to better outcomes for patients.

You will be offered a Diabetes Annual Checks appointment around the time of your birthday. Please bring a first morning urine sample with you and our Healthcare Assistant will process this along with your blood tests, blood pressure, weight and foot screening.

The results will be collated by one of our Diabetes GPs. You will receive a booklet detailing your results, together with a form to aid care planning and a recommendation letter detailing next steps for action or discussion.

The recommendation letter will detail the best member of our healthcare team for you to organise a Care Planning Appointment. This may be with a pharmacist, practice nurse or GP and could be by telephone or face to face. Decisions about treatment, support and when more tests are due will be agreed at this appointment.

Non NHS Services

Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

  • Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
  • Insurance claim forms
  • Letters to support travelling with medication
  • Private sick notes
  • Vaccination certificates

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

Charges are clearly displayed in the reception area.

Travel Vaccinations

If you require any vaccinations relating to foreign travel you need to make an appointment with your local community pharmacist to determine which vaccinations are required.

Vaccinations for Travel are no longer provided by the practice.
